To activate it withon 15 days I miust reply with the bottom box filled in which asks fro my birth day, month and year and time. I also must include $15.00 adsn my email address. in case they need to reach me. and they will also send me a small gift in appreciation as well. the Nominal $15.00 fee is for the release and confirmation of the assistance. it is the only fee associated with the valuable assistance. They also instruct you to put airmail postage of $.94 or use 3 first class stamps on the reply envelope to ensure delivery.
