A 'W-915' form apparently originating from the U.S. The 'North American Headquarters' at PO BOX 11046, Ft Lauderdale, FL 33339-1046, USA.

It suggests that the recipient will recieve a huge prize if they send a twenty pound payment to the above mentioned U.K. address. Rubbish.
