Illegal and unauthorised withdrawal from my debit card

I bought a coach ticket and was asked to feel out my details to claim back 15pounds, which i never claimed back and never used the service to any of my benefits.to my greatest surprise, i realised that my time rewards had used these details to fraudulently take money from me, on my lloyds tsb card what am i paying for? NOTHING!

This is so annoying..i'm totally angry and disappointed.please, i need me money back..acct..
